HCC 134 – Dialysis Status

Renal dialysis is among the most costly conditions in the healthcare. Dialysis status is commonly left out of provider assessments, thereby missing an opportunity to document the extensive benefits and medical resources utilized in providing care.

To meet coding requirements for dialysis status, the intent of the face-to-face encounter needs to be clearly documented.

Hospitals may admit patients seen as outpatients in the provider’s office or those scheduled with a dialysis center for the sole purpose of treatment. Providers should report when kidney disease, renal failure, nephritis, or other kidney diseases lead a patient to renal dialysis, in addition to the administration of treatment diagnosis code.

ICD-10 codes for dialysis in HCC 134 range from status, fitting and adjusting, to testing, complications, and reactions.

 

Status – The coding and documentation must support whether the patient is medically compliant with the treatment.
