HCC 134 – Dialysis Status
November 3, 2020Renal dialysis is among the most costly conditions in the healthcare. Dialysis status is commonly left out of provider assessments, thereby missing an opportunity to document the extensive benefits and medical resources utilized in providing care.
To meet coding requirements for dialysis status, the intent of the face-to-face encounter needs to be clearly documented.
Hospitals may admit patients seen as outpatients in the provider’s office or those scheduled with a dialysis center for the sole purpose of treatment. Providers should report when kidney disease, renal failure, nephritis, or other kidney diseases lead a patient to renal dialysis, in addition to the administration of treatment diagnosis code.
ICD-10 codes for dialysis in HCC 134 range from status, fitting and adjusting, to testing, complications, and reactions.
Status – The coding and documentation must support whether the patient is medically compliant with the treatment. |

Choose the type of dialysis |
Identify the status of dialysis therapy |
Z49.31 Encounter for adequacy testing for hemodialysis |
Z99.2 Dependence on renal dialysis |
Z49.32 Encounter for adequacy testing for peritoneal dialysis |
Z91.15 Noncompliance with renal dialysis |
Fitting/Adjustment (including insertion for later use) |
Z49.01 Encounter for fitting and adjustment of extracorporeal dialysis catheter |
Z49.02 Encounter for fitting and adjustment of peritoneal dialysis catheter |
Testing – Documentation should directly relate dialysis treatment to the testing. |
Z49.31 Encounter for adequacy testing for hemodialysis |
Z49.32 Encounter for adequacy testing for peritoneal dialysis |
Complications and Reactions – When selecting these codes, make sure to add the 7th-digit character to identify if the encounter is: |
T85.691_ Other mechanical complication of intraperitoneal dialysis catheter |
T85.71X_ Infection and inflammatory reaction due to peritoneal dialysis catheter |
T82.818_ Embolism due to vascular prosthetic devices, implants and grafts, initial encounter |
T82.828_ Fibrosis due to vascular prosthetic devices, implants and grafts, initial encounter |
T82.838_ Hemorrhage due to vascular prosthetic devices, implants and grafts, initial encounter |
T82.848_ Pain due to vascular prosthetic devices, implants and grafts, initial encounter |
T82.858_ Stenosis of other vascular prosthetic devices, implants and grafts, initial encounter |
T82.868_ Thrombosis due to vascular prosthetic devices, implants and grafts, initial encounter |
T82.898_ Other specified complication of vascular prosthetic devices, implants and grafts, initial encounter |
