Health Alliance partners with EviCore Healthcare to manage a subset of our Prior Authorization required codes. Effective 1/1/2025 select codes that previously required prior authorization were removed from prior authorization requirements. The codes removed were in the categories of Cardiology (ECHO), Sleep, and Radiology/Nuclear Medicine. The list of specific codes removed from prior authorization are noted in the table below.
